Becoming a Preceptor Process

Processing new preceptor application forms - beginning with Contacting the ELP Office to Onboarding with HR to Copying the information gathered to the admin2k preceptor table.

Becoming a Preceptor Process

  1. Directions on the https://experiential.sop.pharmacy.wisc.edu/ website provide the ELP Office email address for the potential preceptor to request access to the Becoming a Preceptor Application form.
  2. ELP office (Tina) sends an email to the potential preceptor that includes a link to the application (https://apps.pharmacy.wisc.edu/clerkshipapply/apply.php) and the code required to access the form.
  3. Potential preceptor fills out the form and submits.
  4. ELP office (Tina) verifies the WI pharmacy license and then clicks on 'Admin Verified' in the Office section of the form or 'Decline's the application .
  5. ELP director (Marty) reviews the form and completes the office section of the form (determines the kind of courses the preceptor can take and whether or not the site requires a site visit) and then clicks on 'Director Verified' or 'Decline's the preceptor's application.
  6. Once verified, ELP admin (Tina) sends an email to the potential preceptor providing directions on how to send their SSN to the SoP HR department.  Once this email is sent, the ELP office (Tina) will click 'Yes' for 'SSN Email Sent to Preceptor? Or not needed?' so HR will receive an email reminder that an application is ready for review. 
  7. HR (Allison) gains access to the application and once the SSN is received and the preceptor added as an employee of the university, adds the EMPLID to the office section of the form.
  8. HR (Allison) also generates the new preceptor's NetID.  The NetID is put into the following BOX folder: https://uwmadison.app.box.com/folder/147558967595 .
  9. Information gathering is now complete and the ELP office (Tina) can use the 'Move Preceptor' button to either add the new preceptor record to the STAR preceptor table or update and activate an older record if one exists.  More information will need to be added by the ELP office (Tina) as it becomes known. (ie. RPHh status, whether or not they are special graders, etc.) ELP officer (Tina) also uses 'Site/Preceptor/Course Link' to connect the preceptor to a site and course(s).
  10. ELP office (Tina) obtains the NetID from the BOX folder mentioned above (https://uwmadison.app.box.com/folder/147558967595)  and updates the Preceptor record in STAR.  ELP office (Tina) also emails the new preceptor explaining how to active their NetID.
  11. ELP office (Julie) will be notified if the new preceptor/site requires a site visit.  Once the site visit has been scheduled, the record should be marked as done to prevent more email reminders.
