Policy
At the Center for Healthy Minds, the role responsible and accountable for making
sure that business associate agreements are in place before CHM enables a service
provider to begin to create, access, store or transmit ePHI on CHM’s behalf is CHM’s
most senior Financial Specialist.

Definitions
A. BAA = Business Associate Agreement
B. CHM = Center for Healthy Minds
C. ePHI = Electronic Protected Health Information
D. SJD = Standard Job Description

Procedures
The implementation, execution, and maintenance of this policy is the responsibility
of the Center Administrator and/or the supervisor of CHM’s most senior Financial
Specialist.
