CHM Undergrads - Departure Checklist

Departure checklist file and instructions for completing the checklist when a student leaves the lab (for supervisors to complete).

Introduction

Supervisors must complete the “Departure Checklist for Supervisors of Undergraduates” document before any student (research credit OR hourly) departs the lab.

The last day a graduating undergraduate can work (unless they have registered for graduate-level classes at any institution): 

  • Summer 2024: May 18, 2024

Key Fob Reminder: Please return the key fobs of undergraduates as soon as they have departed CHM. If you have fobs from past undergraduate departures, please return them immediately. The fobs can be returned to Sarah Keib directly or placed in a sealed envelope (write undergrad's name on front) and deposited in the box on her desk labeled "return keys here." If you have any questions about this process, please contact Sarah (keib@wisc.edu). 

Filling Out the Departure Checklist

  1. Click the “Undergraduate Departure Checklist for Supervisors (PowerForm)” link (above) to launch the DocuSign PowerForm.
  2. Enter your name and email. You do not need a DocuSign account to fill out the form.
  3. Complete the form.
    1. To finish the form later, click "Finish Later." Your progress will be saved, and you will receive an email at the address you provided with a link to finish the form later.
  4. When you are finished, click "Submit". The form will automatically be sent to Undergraduate Administration.

Complete the process listed above for each individual Departure Checklist that you need to complete. The PowerForm link generates a single Departure Checklist.
