Policy Regarding Institutional Certification of Eligibility for NRSA F31's to Promote Diversity

In February 2008, the NIH issued NIH Guide Notice Number: NOT-OD-08-039 clarifying the requirement for a letter from the applicant institution certifying that the applicant for this program belongs to one of the eligible groups as defined in the application instructions. This letter should be inserted in the application immediately before the checklist (at the end of the Sponsor’s/Co-sponsor’s information).

This document describes the process for fulfilling this requirement for applicants from the University of Wisconsin–Madison.


If a student in your program informs you that they intend to apply to this fellowship program, refer to the various options for verifying eligibility listed below to determine which option is appropriate. Some of the documentation may take some time to obtain, so this should be done as early as possible. Once you have ensured that you can certify eligibility according to one of these options, the program may prepare a letter based on the sample provided below. The letter needs to be signed by the department or program chair,and any supporting documentation is retained in the department/program files.

Means of Verifying Eligibility

  • Graduate School Application-ethnic heritage (page 1), participation in TRIO, McNair or first-generation college student (Funding Form), other information contained in personal statement
  • Visa from McBurney Disability Resource Center
  • Verification from undergraduate institution of receipt of offer from one of the loan programs listed in the eligibility section of the Funding Opportunity Announcement (FAO) or funding specifically based on race/ethnicity, disability, income level, or first generation student.
