  1. Student wants to obtain a letter of referral for gender confirming hormones
    1. Triage/Access or HB (May refer to GIC from any service when student requests letter of referral for hormones
    2. Gender Identity Consultation
    3. Is letter of referral appropriate?
      1. Yes, letter of referral is appropriate
        1. Provider writes letter, reviews with student, includes in chart
          1. Care at UHS medical Services?
            1. Yes, care at UHS
              1. UHS Primary Care (See Medical Services section for flow chart)
            2. No, care not at UHS
              1. Community Prescriber (Student to sign ROI at HIM Letter faxed to prescriber)
      2. No, letter of referral is not appropriate
        1. Answer questions provide resources, refer as relevant
  2. Student wants to discuss/explore gender identity; talk about whether gender confirming hormones are right for them; obtain help/resources for gender transition; learn about the physical changes that generally occur with gender confirming hormones
    1. Triage/Access or BH (May refer to GIC from any service when student requests letter of referral for hormones)
    2. Counseling scheduled consultation with privileged provider
    3. If letter requested, Gender Identity Consultation
  3. Student wants to initiate gender confirming hormones; continue gender confirming hormones; talk to a medical prescriber about specific physiological changes they may expect with gender confirming hormones
    1. UHS Primary Care (See Medical Services section for flowchart)
