Please note, the below forms must be completed and approved at the department level before being submitted to hrstaff@ls.wisc.edu. The L&S HR SME team will complete the Finance and HR approvals.

Base Compensation Change Form: Complete this form if there is a request to change an employee's base salary because there was a change in responsibilities and title, change in unique responsibilities, market competitive adjustment, market retention adjustment, parity, or above and beyond performance.

One-Time Payment (OTP) and Period Activity Pay (PAP) Request Form: Complete this form to request a One Time Payment or a Period Activity Pay. Always complete the employee information and department approval section and only complete the OTP or PAP section as it relates to the request. Each request is defined on the form under select the request type. OTP request includes faculty staff award, hiring bonus, moving expenses post hire, referral bonus, and retention bonus. PAP request includes continuing education courses, coverage, event speaker, lecture, lecture (not for credit), mentoring, other service-related payment, and overload.

Temporary Work Adjustment Form: Complete this form when there is a temporary change in employee duties and/or to recognize an employee for taking on higher-level responsibilities on a temporary basis.

Position Review Form: Complete this form if the employee's responsibilities have changed, or will be changing, since the position was last reviewed and you believe a new title is warranted.
