PMDP Probation and Evaluation Cycle Charts

The PMDP Probation and Evaluation Cycle Charts detail when an employee's 30 Day Conversation, 3 Month Midpoint Probationary Evaluation, and 6 Month Probationary Final Evaluation will take place based on the employee's hire month. It also explains when the employee will join the SMPH annual review cycle after their probation.

Our general guidance is that if an individual had a probationary summary evaluation within the three months prior to the end of the next performance evaluation range (December 31st for mid-points or June 30th for summary), they do not have to do another evaluation at that point of our annual review cycle. That individual will start in our annual review cycle the following review. If someone's probationary summary evaluation was not completed within the prior three months, they will need to have a review at that point in time. Please see the chart below for examples:
