Update to UW-Madison Qualtrics Survey Background

On May 15th, the official UW background for surveys created in Qualtrics will be updated to match the new UW website theme.
The default background used for surveys created and distributed through Qualtrics is based on a layout and color scheme (a.k.a. "theme") developed by UW Marketing. This official UW Theme has been updated, and the changes will pushed out to Qualtrics on May 15th.
For reference, the old theme is shown below, alongside the new theme:

Surveys created prior to May 15th will continue to display the original theme, though the survey owner will be able to switch their survey to use the new theme if desired. Please note that once a survey has been manually switched from the old theme to the new theme, you will not be able to re-apply the old theme.
This change only affects the surveys themselves, and does not apply to the interface when working in Qualtrics to create and edit surveys.
