Accessing 1098-T Tax Forms

This document details how UW-Madison students can obtain their 1098-T tax form online.

Accessing the 1098-T form portal for the years 2020-2023

  1. Log in to your MyUW portal here.

  2. Click on the Student Center application.

  3. Click on the Financial Account tile.

  4. Select "View 1098T" from the left-hand list.

  5. Click the "Grant Consent" button toward the bottom of the page.

  6. The years under the "Tax Year" column should now be hyperlinks. Click the tax year for which you want to open, view, download, or print your 1098-T tax form.

 Accessing the 1098-T form portal for the year 2019

  1. Log in to your MyUW portal here.

  2. Click on the Student Center application.

  3. Click on the Financial Account tile.

  4. Select "View 1098T" from the left-hand list.

  5. Click on the "Access My 2019 1098-T" button toward the bottom of the page.

  6. A new page should open with a link for the 2019 1098-T form; click this link

    • Note: In order for the hyperlink to load, you must ensure that your pop-up blocker is turned off

Accessing the 1098-T form for 2018 (and previous years)

The 1098-T form for 2018 is unavailable online due to a vendor change. If you did not receive a 1098-T form by mail in 2018 or you require an additional copy, please contact the Bursar's office by phone at (608) 262-3611 or by email at tuition@bussvc.wisc.edu.

For 1098-T forms for years previous to 2018, you can request a certificate online.

Logging in to the 1098-T Form Portal (Secure e-Statement Website)

  1. Navigate to the Secure e-Statement Website.

  2. If you have previously accessed your 1098-T form using this service, you can log in using your campus ID number as the Student ID and the password you created during the enrollment for the service.
  3. Otherwise, if you have not previously accessed your 1098-T form, you need to "enroll" in order to access the form. Begin the enrollment by clicking on "Click Here to Enroll."

  4. Enter your first and last name in the respective fields.

  5. Enter the last four digits of your social security number in the Security Phrase field.

  6. Enter your primary @wisc.edu email address in the email address field. Primary addresses are usually of the form netID@wisc.edu or first.last@wisc.edu. If you are unsure of your primary address, please refer to Microsoft 365 - Change your Primary Address.

  7. Enter your campus ID number in the Student ID field.

  8. Enter your desired password in both password fields. Passwords must contain an uppercase letter, lowercase letter, number, and special character.

  9. Click on the box acknowledging your consent to the use of electronic records for the form.

  10. Click on Enroll to finish the enrollment. If the information was entered correctly, you will be signed in to view your available forms. If the screen refreshes and the information fields are blank, one of the provided inputs was incorrect and you will need to start again.
