(Madison) Routing slip template
These are the templates for the routing slips used by UW-Madison to route campus items. The "Return" slips should be photocopied on salmon colored paper, the "Route for Hold" slips should be photocopied on green paper
If a UW-Madison library is not listed on the routing slip this may be an indication that the library does not receive red box delivery. Use campus mail or another mechanism to return the item to its home location.
