Requesting access to Microsoft Azure OpenAI Services

Ensure you have an Azure Subscription
You will first need an Azure account (Note: Microsoft calls Azure accounts "subscriptions"). If you have an existing Azure subscription, you can request to add the OpenAI service to this subscription.
If you don’t already have an Azure subscription and would like to use the Azure OpenAI service, follow the instructions in Requesting an Azure Subscription and indicate in the notes field of the form that you will be accessing the Azure OpenAI Service.

Request access to Azure OpenAI Services
By default, UW–Madison Azure subscriptions do not include access to Azure OpenAI Services (e.g., GPT 3.5 and GPT 4 models).
To request access to Azure OpenAI Service:

Open the Apply for access form from Microsoft’s Azure OpenAI Service page

In this form,

Enter your Azure Subscription ID (This is usually a long alphanumeric string, e.g., abcdef01-2345-6789-Oabc-def012345678)

When asked for your email address, enter your UW-Madison e-mail address in the format, netid@wisc.edu

When asked for information about a Microsoft contact, use UW–Madison’s Customer Success Account Manager at Microsoft, Justin Lewis, lewis.justin@microsoft.com

Microsoft is usually able to approve requests to use Azure OpenAI Service in about one week.
