Former Employees - Accessing Earning, Leave, Tax, and Benefit Statements

Current and former employees can view and download statements in Workday. You can also request additional copies of statements from UW Shared Services.

Log in to Workday

Former employees whose positions ended after July 1, 2025 must use a Workday username and password, not your UW-Madison NetID. You can log in for up to two years after leaving the university. Please check your email or reach out to gethelpuw@support.wisconsin.edu to get your username and password.

Current employees and former student employees who are still enrolled at UW-Madison can access Workday through MyUW using their NetID.

  • No, I am a former employee
    • When did your position end?
    • Before July 1, 2025
      • Since you left UW-Madison before Workday launched, unfortunately you are not eligible to access Workday. You can still get copies of your statements from the contacts listed below. 
    • After July 1, 2025
      • Do you have a Workday username and password? This will likely be different than your NetID.
      • Yes
      • Yes, but they aren't working
        • If you need help with your username, please contact gethelpuw@support.wisconsin.edu for assistance. 


          To reset your password: 
          1. Go to the Workday login page.

          2. Click Forgot Password?

          3. Enter your Email address (enter the email address listed as home contact information in your Workday account). 

          4. Check your inbox for an email with a Reset Your Password link.

      • No
        • When your termination was processed, your Workday username and password were emailed to you automatically. They were sent to your personal email address on file, or to your campus email if no personal address was available.

          If more than 90 days have passed or you cannot access those emails, contact gethelpuw@support.wisconsin.edu to request your Workday username and password. They will also provide the direct login link and can help with any additional issues.

Statement type

Earning statements Contact your former payroll office or contact UW Shared Services' Service Operations help desk.
Tax statements (W-2, 1042-S, Fellowship Tax letters)

You must submit a request using the Duplicate Statement Request Form.

Documents from 2017 and earlier are $10 per form, per year.

Leave and Benefit statements Contact UW Shared Services' Service Operations help desk.
