Canvas Request Forms
Canvas Non-Credit Course Request
Request a new noncredit Canvas course. Requests may take 1-3 business days to be fulfilled.
Canvas Sub-Account Admin Request
Request any of several account-level permission sets for UW-Madison instance of Canvas.
Canvas Commons Group Request
Request a new group to share materials in Canvas Commons.
Canvas External/Third-Party Integration Request Form
Request an integration of a third-party tool with Canvas. See Canvas - Enabled Application Configurations [UW-Madison].
Canvas Access Token Request Form
Request a Canvas Access Token. Access tokens provide access to Canvas resources through the Canvas API. Using the Canvas API, the person or program with the access token can access the same Canvas resources as you.
If approved, a UW-Madison Canvas admin will generate an access token on your behalf. You'll receive a follow up email with a link to a secure note in 1Password.
Kaltura Request Forms
Bulk ownership change request
- Kaltura MediaSpace users can change the ownership of their media content or download items one at a time. However, if you need to perform these actions on a large number of items, you can fill out one of the forms below to request one of these changes in bulk. It may take up to five business days for a Kaltura administrator to fulfill your bulk change request.
- Ownership of media content must be transferred to an individual with a valid, active NetID. Service accounts cannot be used for bulk ownership change requests.
Bulk video download request

The Bulk Download process is limited to 500 media items.

After the process completes, it will generate an email that is sent to the requested user. The user is responsible for manually downloading each video from the links generated.

The links are active for approximately 24 hours.
Kaltura Group Request

Kaltura groups allow collections of users to manage Kaltura media and channels. More information on Kaltura groups and links to forms on requesting one is available at Kaltura - Groups [UW-Madison] .
Request to add co-editor, co-publisher, or co-viewer
Request restoration of deleted media

Learn@UW can request that deleted Kaltura media be restored for approximately a week past its deletion. Remember to keep original copies of any media you upload to Kaltura MediaSpace.

Handshake Request Forms
Regular Accounts
Handshake Accounts for Pre-2014 Alumni
All enrolled, degree-seeking students, capstone students, and degreed alumni have access to Handshake. However, if you meet one of the two conditions below, you will need to complete a form to request an account. Use this form if you earned a degree before 2014.
Handshake Accounts for Postdoctoral Students
The Office of Postdoctoral Affairs provisions current UW-Madison postdoctoral students with Handshake student accounts. If you are a postdoctoral student, fill out this form to request an account.
Students associated with the Division of Continuing Studies, including special students, visiting students, and exchange students, are not considered degree-seeking students; therefore, are not authorized access to Handshake.
Administrative Accounts
Handshake Administrative Account Request
Use this form to request administrative accounts or student test accounts.Administrative accounts are for UW employees needing back-end access to view and/or manage student profiles/activity, career events/fairs, employer information, and/or marketing and communications with students and employers. This account is typically for career services staff members. When administrative accounts are requested, users select the role groups/permission levels they need. To learn more about role groups/permission levels, review Handshake Role Groups for Administrative Accounts.
Student test accounts are available for UW employees who are NOT in a career services role and do not require back-end administrative access. This account allows you to see the student/alumni interface (aka what students and alumni see). All administrative accounts also receive a student test account. For the question “What type of role best describes you?”, on the request form, select “Professional staff member, faculty, or instructor in a non-career services role”.

Form not loading properly?
If your form does not load properly:

Open the form in a Private Browser Window.

Try accessing the form using the Mozilla Firefox web browser.
