REDCap: Image Map External Module
The Image Map External Module (EM) in REDCap allows respondents to replace standard data entry fields (like text boxes, radio buttons, or checkboxes) with an interactive, clickable image. Users can click specific regions on an image to select values, which is particularly useful for clinical assessments such as body pain maps, dental charts, or smiley-face pain scales.
Setup Instructions
Any user with Project Design & Setup access can enable the Image Map module. The External Module (EM) section is under Applications. Select 'View available modules'. Look for the Image Map Module and request activation. Once enabled by REDCap Administrator, the EM will be displayed in the 'Currently Enabled Modules'. Enter the form name you want to add the Image Map to. Please note required field types are provided for adding each image map below.
Image Map EM Configuration.
To display one of the image maps in a survey or data entry form, add a new field and include one of the options of the @IMAGEMAP listed below in Action Tags / Field Annotation box. See below for a complete list of image maps available to choose from.
The Image Map module is triggered by adding the @IMAGEMAP action tag to a field's "Action Tags/Field Annotation" box, followed by the specific map name (e.g., @IMAGEMAP=CHOIRBODYMAP_MALE).
Each region of an image map is associated with a coded value, for example, the "Wrist (front-right)" of the male body diagram is linked to the coed value "f19". To find a particular coded value for a body part, please refer to the coded values presented below for each option of @IMAGEMAP action tag. After selecting multiple body parts, the field containing the action tag @IMAGEMAP will have as a value a string of comma-separated coded values, e.g. "f36,f17,f18,f21".
Best Practices and Limitations
Modifying coded values in option lists in Image Map EM is discouraged. The Image Map module relies on the original coded values to map user selections to specific regions of the image. Changing these values will disrupt this mapping and cause the image selection to function incorrectly. The field labels for options may be updated as needed, as this does not affect functionality. For example, updating a response label from “Head” to “Upper Head” is acceptable, but changing the coded value (e.g., from 1 to 9) is not supported.
The @NONEOFTHEABOVE action tag is not compatible with the Image Map module and will not properly clear image-based responses.
Please keep in mind:
- Additional images cannot be added to the Image Map module
- Existing images in the EM cannot be modified
- Underlying configuration (including coded values, color assignments, clickable regions, and selection order) must remain unchanged

The 66 Swollen / 68 Tender Joint Count can be used to record the number of tender and swollen joints in patients with inflammatory arthritis e.g. psoriatic arthritis, rheumatoid arthritis. These counts can be used in calculated fields to generate disease activity scores in combination with other variables e.g. DAPSA, DAS28 scores.
