| Eligibility Version History | Lists the eligibility questionnaires created for this protocol and their current status. After creating a new version and returning to the View mode, each version will have a hyperlink. A “Delete Latest Version” button is available only if the latest version of the questionnaire has not been used for enrolling any subjects. Version No.: Displays the order the Eligibility questionnaires have been created. Version Date: Displays the date the version was first created. Subject Count: Displays the number of subjects screened using this version of the eligibility questionnaire. Status: Status is either In Progress or Released. A “Released” button is provided on the update screen. Once a questionnaire is released, the prior version will no longer be used for screening subjects for eligibility. |
