This table lists the subjects that have an eligibility record. |

If eligibility was determined by an online questionnaire, data will appear in the Form Version field. If the Form Version field is blank, eligibility was entered directly in the Subject Console.
- Subject MRN: Selecting the Subject MRN hyperlink will open the Subject Console-Eligibility tab for that subject where status change or updating is available (with the correct privileges).
- Subject Initials: The first and last initials of the subject’s name.
- Pre-Screening Consent Signed Date: Displays the date the subject signed the consent form as indicated in Subject Console > Consent.
- Status: The eligibility status of the subject.
- Reason Withdrawn: The reason the subject has withdrawn their consent. This information is captured in the Subject Console > Consent tab.
- Form Version: Version of the Eligibility Questionnaire if applicable.
- Entered By: The person who entered the eligibility record.
- Entered Date: The date the eligibility record was entered.
