| Sequence Number | The unique study number that is assigned to each subject. If the sponsor, cancer center, or research group assigns a study number, enter that here. This number should not contain any identifying information such as initials and should not contain any characters or symbols other than those that are alphanumeric. | Required To have OnCore assign a unique number to each subject, activate Auto-generated Sequence Number in the PC Console > Main > Management Tab before the first subject is accrued. Note: If OnCore: Automated Sequence Number is set to yes, the sequence number will appear only after the On Study Date has been entered and “Submit” is clicked. |
