Acknowledge Reportable Event

This is an activity in ARROW that allows the IRB staff reviewer to acknowledge receipt of a reportable event that does not require further IRB review. Upon acknowledgement, the study will return to the approved state.

The Wisconsin Idea is one of the longest and deepest traditions at the University of Wisconsin-Madison and serves as the guiding principle in all that we do. Simply put, the Wisconsin Idea states that the education a student receives on our campus should influence people’s lives beyond the boundaries of the classroom to make the world a better place.

Through the review of an applicant’s involvement, essays, and letters of recommendation, this item rates the applicant’s eagerness to explore and embody the principles of the Wisconsin Idea and their potential to enrich our campus community. 

Valid values in hierarchical order are:
Extraordinary

Few are given this highest rating. Students in this category have shown evidence of interest in making an impact on their community through a UW-Madison education AND shown evidence of currently making an impact on their school/community. This could include:

  • Reflection on work they've already done to make an impact on their community/society and how they will continue to do so
  • An action or impact plan to advance change that is inspired/informed by their lived experiences, observations, or intellectual pursuits
  • Extraordinary commitment to furthering knowledge through discovery, leadership, and problem solving to assist in addressing societal challenges
Exceeds Expectations

These students show evidence of a genuine interest in making an impact on their community through a UW-Madison education. This could include:

  • Making an impact in their community or society at large
  • Contributing to research/discovery to serve the greater good
  • A desire to address societal challenges
Meets Expectations

This is the most common rating in our applicant pool by far. These students show involvement in high school and/or interest in involvement as a student at UW-Madison though do not rise to the level of the first two categories.

Average These students may have limited involvement and/or share minimally as to how they will become involved as a student at UW-Madison.

    Approved with Administrative Hold

    This is a state in ARROW.  Studies in the Approved with Administrative Hold state have been reviewed and approved by the IRB; however, additional administrative documentation is required before final approval is granted (e.g. submission of a Certificate of Confidentiality). 


    Auto-Match Opportunity (WiSH)


    Awaiting Correspondence

    This is a state in ARROW.  Studies in this state have undergone IRB review, a meeting decision has been recorded and the IRB staff can draft correspondence for the study team. 


    Awaiting Scientific Review

    This is a state in ARROW.  Studies in this state have been triaged by the IRB and determined to require scientific review.  In this state, the Scientific Review Committee (e.g. CTRC PIR, SRC, PRMC) will review the study and indicate the outcome of their review.   


    Award Period (WiSH)

    The Award Period is the term(s) you intend to pay the student their scholarship/award.

    Category (WiSH)

    Within the WiSH system, students are assigned categories to show their status/progression through the system. Students can have different categories within the General Application, Auto-Match, Apply-To and Post-Acceptance grids depending on where they are in the scholarship process. Below are a list of the available categories within WiSH. Please note: Applicants may not see the same category names as administrators. If applicants see a different name, it will be indicated in the table.

    Apply-To Student Categories

    Accepted - Paid Out From Payroll Applicants and Reviewers (displays as "Accepted") Instead of being paid through the Bursar's Office, the applicant's award was processed and paid to them through Payroll.
    Accepted – Post to Financial Aid for Disbursement Applicants and Reviewers (displays as "Accepted") Sends the student's award to their financial aid offer in the Student Information System (SIS) while also allowing the student to continue to complete Post-Acceptance.
    Accepted - Sent to Payroll/Glacier Applicants and Reviewers (displays as "Accepted") This status is used to track which awardees with visas you or your business office have sent to the Glacier Team. Awards in this status will NOT be sent to SIS. If a student's award was previously sent to SIS using a different status and their was moved to this status, their scholarship in SIS will be removed
    Applicant Declined Applicants and Reviewers The applicant has declined the award.
    Cancelled Administrators only The scholarship administrator no longer wishes to offer the student the award.
    Drafted Applicants The application has been started/opened but the applicant has not yet submitted the application.
    Ineligible – Drafted Applicants and Reviewers (displays as "Drafted") A way you can mark a student who has started an application as ineligible without notifying the student.
    Ineligible – Submitted Applicants and Reviewers (displays as "Submitted") A way you can mark a student who has submitted an application as ineligible without notifying the student.
    Offered Applicant and Reviewers The scholarship administrator has offered an award to the applicant. The applicant can now accept or decline this offer. If set to immediately, communications notifying the applicant of their offer will be sent as soon as they are moved to this category. If set to upon approval, the communications notifying the applicant of their offer will be in scholarship administrator's message queue to be approved. 
    Plan to Offer Reviewers The scholarship administrator has encumbered funds by creating an offer for the applicant. However, the applicant cannot see the offer. To them, their category is still "Submitted." Additionally, the applicant has not been sent a communication notifying them of their offer.

    Awarded by Bursar Upload Applicant (displays as "Awarded") The scholarship administrator paid the applicant's award using the Bursar Scholarship Upload process instead of using WiSH functionality.
    Awarded - Paid Out From Payroll Applicants (displays as "Awarded") Instead of being paid through the Bursar's Office, the applicant's award was processed and paid to them through Payroll. When in this category, students cannot adjust their P.A. answers.
    Awarded - Sent to Payroll/Glacier Applicants (displays as "Awarded") This status is used to track which awardees with visas you or your business office have sent to the Glacier Team. Awards in this status will NOT be sent to SIS. If a student's award was previously sent to SIS using a different status and their was moved to this status, their scholarship in SIS will be removed. When in this category, students cannot adjust their P.A. answers.
    Cancelled Applicants The scholarship administrator has cancelled the award and future disbursements (if applicable).
    Deferred Applicants The scholarship administrator has deferred the payment for another term (for renewals specifically).
    Drafted Applicant The applicant has started to complete the P.A. questions but they have not submitted their responses.
    Final Disbursement Applicants The scholarship administrator marks this award payment as the last payment of a renewable scholarship. This category works similar to "Awarded" in that the scholarship will be sent to SIS for payment.
    Pending Administrators Only The initial category of the P.A. grid. When the scholarship administrator first encumbers an award for the applicant in the Applications grid, the applicant appears in the P.A. grid with this status.
    Requested Applicants The applicant has been asked to completed P.A. Applicants automatically move to this category when they accept the award.
    Submitted Applicants and Reviewers The applicant has completed all necessary/required parts of P.A.

    General Application and Conditional Application Categories

    CELL WALL

    Fibrous structure that provides rigidity to the plant. The cell wall is composed of digestible fibrous carbohydrates (cellulose; hemicellulose and pectin) and an indigestible phenolic compounds (e.g., lignin and tannin).


    Changes Requested - Scientific Review

    This is a state in ARROW.  Studies in this state have undergone scientific review and changes were requested by the scientific review committee.  In this state, the study team can either 1) respond to the committee request so that it returns to the Awaiting Scientific Review state for additional review, or 2) withdraw the application. 


    CHEW (to)

    To crush or grind (food) in the mouth by continued action of the teeth with the help of the tongue (syn to masticate).


    Conditional Application (WiSH)


    Consultant Review

    This is an activity in ARROW that allows the IRB staff reviewer to document nature of the consult and any relevant information about the outcome of the consult.


    Continuing or New Student

    A value assigned to a student based on their admissions and enrollment at the university. It is not a standard field in SIS and is thus calculated using a mixture of enrollment data and admissions data. The value “continuing” is assigned to students who are ‘eligible to enroll’ or ‘enrolled’ in the SIS affiliation table. The value “new” is assigned to applicants who have applied for admission, been admitted or indicated they intend to enroll.

    Timing is an important consideration if using this field in terms of your scholarship cycle.  A ‘New’ applicant will become a ‘Continuing’ student when matriculation occurs.  For Admit Terms, matriculation occurs:
    • Admitted for summer term = matriculation occurs in late March
    • Admitted for fall term = matriculation occurs in May
    • Admitted for spring term = matriculation occurs in mid-November

    Drawer

    Documents in ImageNow are stored in drawers; the drawer of a document is a document key with unique properties: user access to view documents in drawers can be limited, views/filters typically focus on a single drawer, and the drawer key for a document is required.


    DRY COW

    A non-lactating cow. The dry period is the time between lactation, when the cow is not secreting milk.


    Encumbered Funds (WiSH)

    Encumbered Funds are the total amount of aid/funding an application has been offered or awarded.

    This is an activity in ARROW that allows the IRB submission manager to forward a submission in the IRB withdrawn state to the Changes Requested -Scientific Review state.


    Forward to Deferral Response Pending

    This is an activity in ARROW that allows the IRB submission manager to forward a submission in the IRB withdrawn state to the Deferral Response Pending state.


    Forward to Expedited Reviewer

    This is an activity in ARROW that allows the IRB staff reviewer to forward an application to an expedited reviewer.  This activity should be used when it is determined that expedited review is the most appropriate review process for the application. 


    Forward to Faciliated Reviewer

    This is an activity in ARROW that allows the IRB staff reviewer to forward an application to a facilitated reviewer.  This activity should be used when it is determined that facilitated review is the most appropriate review process for the application.


    Forward to IRB Administrative Review

    This is an activity in ARROW that allows the IRB submission manager to forward a new submission to administrative review when scientific review is not required for the new submission. The activity requires attestation from IRB submission manager  that scientific review is not required. 


    Forward to IRB Pre-Meeting

    This is an activity in ARROW that allows the IRB submission manager to transition a submission out of the PRS review state back to the IRB Pre-Meeting state in preparation for review at a convened IRB meeting.  


    Forward to IRB Pre-Meeting

    This is an activity in ARROW that allows the IRB submission manager or the IRB staff reviewer to forward a submission currently in the PRS review state to IRB Pre-Review.


    Forward to IRB Review of Modification Response

    This is an activity in ARROW that allows the IRB staff reviewer to forward a submission to the IRB for review of a modification response from the study team.  This activity can be used when the study team's response requires additional review by the full IRB.


    Forward to Issues Pending

    This is an activity in ARROW that allows the IRB submission manager to forward a submission in the IRB withdrawn state to the Issues Pending state.


    Forward to Modifications Requested

    This is an activity in ARROW that allows the IRB submission manager to forward a submission in the IRB withdrawn state to the Modifications Requested state.


    Forward to Pre-Submission

    This is an activity in ARROW that allows the IRB staff reviewer to forward a submission back to the Pre-submission state.  This activity can occur in Pre-review, In Expedited Review and In Exempt Review states.  This activity can be used when the application has been triaged incorrectly (e.g. requires review by a different IRB or scientific review). 


    Forward to Scientific Review

    This is an activity in ARROW that allows the IRB submission manager or IRB staff reviewer to forward a submission to scientific review.  Applications will be assigned to: 1. CTRC PIR, 2. PRMC, or 3. ICTR SRC. 


    Foward to Exempt Reviewer

    This is an activity in ARROW that allows the IRB staff reviewer to forward an application to an exempt reviewer.  This activity should be used when it is determined that exempt review is the most appropriate review process for the application.


    Foward to Full Review

    This is an activity in ARROW that allows the IRB staff reviewer to forward an application in the expedited review state to full review because the application  requires full review.


    General Application (WiSH)


    Global Warming Potential (GWP)

    An index (i.e., a relative measure) of how much heat a greenhouse gas traps in the atmosphere over a chosen time horizon, relative to that of carbon dioxide. The GWP represents the combined effect of the differing times these gases remain in the atmosphere and their relative effectiveness in absorbing outgoing thermal infrared radiation. Although the most common time horizon is 100 years, GWP have been reported also for time horizons of 20 years and 500 years.


    Hide/Show Errors

    The Hide/Show Errors link appears at the top of each application form. This link checks the submission for any required questions that are not answered.


    High School Rural Designation (WI High Schools Only)

    The designation assigned to an applicant by the Office of Admissions and Recruitment based on a student's High School community. Please note that the designation is only consistently populated for Wisconsin High Schools at this time. The designation values are as follows: 
    • City - Large: Territory inside an urbanized area and inside a principal city with population of 250,000 or more.
    • City - Midsize: Territory inside an urbanized area and inside a principal city with population less than 250,000 and greater than or equal to 100,000.
    • City - Small: Territory inside an urbanized area and inside a principal city with population less than 100,000.
    • Suburb - Large: Territory outside a principal city and inside an urbanized area with population of 250,000 or more.
    • Suburb - Midsize: Territory outside a principal city and inside an urbanized area with population less than 250,000 and greater than or equal to 100,000.
    • Suburb - Small: Territory outside a principal city and inside an urbanized area with population less than 100,000.
    • Town - Fringe: Territory inside an urban cluster that is less than or equal to 10 miles from an urbanized area.
    • Town - Distant: Territory inside an urban cluster that is more than 10 miles and less than or equal to 35 miles from an urbanized area.
    • Town - Remote: Territory inside an urban cluster that is more than 35 miles from an urbanized area.
    • Rural - Fringe: Census-defined rural territory that is less than or equal to 5 miles from an urbanized area, as well as rural territory that is less than or equal to 2.5 miles from an urban cluster.
    • Rural - Distant: Census-defined rural territory that is more than 5 miles but less than or equal to 25 miles from an urbanized area, as well as rural territory that is more than 2.5 miles but less than or equal to 10 miles from an urban cluster.
    • Rural - Remote: Census-defined rural territory that is more than 25 miles from an urbanized area and is also more than 10 miles from an urban cluster.


    HIPAA Waiver

    This is an activity in ARROW that allows the IRB staff reviewer to issue approval for a full HIPAA waiver of authorization, partial waiver of authorization or altered authorization.


    Hold Change for Continuing Review

    This is activity in ARROW that allows the IRB staff reviewer to hold a change application submitted by the research team in order to proceed with review of a continuing review application that has also been submitted concurrently.  In ARROW, with the exception of a personnel change amendment, only one submission will be in the IRB review process at a time.


    Hold Continuing Review for Reportable Event

    This is an activity in ARROW that allows the IRB staff reviewer to hold a continuing review application submitted by the research team in order to proceed with review of a reportable event application that has been submitted concurrently.  In ARROW, with the exception of a personnel change amendment, only one submission will be in the IRB review process at a time.


    Hold for Scheduling: Ancillary Committee Review Pending

    This is a state in ARROW.  Studies in this state cannot be scheduled for an IRB meeting until the Ancillary Committee Review decision has been entered into the system.


    Hold IRB Review - Continuing Review Required

    This is a state in ARROW.  In this state, change applications are being held until a continuing review application has been submitted by the study team and reviewed by the IRB.


    Hold IRB Review - Submission of Reportable Event Pending

    This is a state in ARROW.  In this state, continuing and change applications are being held until a reportable event has been submitted by the study team.  Once the reportable event has been submitted, the continuing or change will transition back to the pre-review state.


    ImageNow Printer

    Software that enables you to "print" files into a .TIFF (tagged image file format); for desktop users, the ImageNow Printer will display as a device in the “Devices and Printers,” “Printers and Faxes” area of the Windows’ Control Panel.


    Import File (WiSH)


    In Exempt Review

    This is a state in ARROW.  Studies in this state have been identified as qualifying for exemption review and have been assigned an exempt reviewer.


    In Expedited Review

    This is state in ARROW.  Studies in this state have been identified as qualifying for expedited review and have been assigned an expedited reviewer. 


    Internal End Date (WiSH)

    The final day on which a student can match to an Auto-Match opportunity. It is also the final day a student's imported SIS data will update in an opportunity's application grid. (The data will freeze at 11:59 PM on this day.) Please note that:

    • Students cannot see the Internal End Date; it is only for administrators.
    • Every opportunity must have an Internal End Date that ends before the start of Cycle Management of the next year (the process of opening the new General Application for the year.)
    • If you change the Internal End Date after it has passed to a date in the future, students will start matching to your opportunity (if an Auto-Match) and student imported SIS data will begin to update again.
    • Internal End Dates should be set as the same day or after the date the Public End Date is set to.


    IRB Administrative Review

    This is a state in ARROW.  Studies in this state will undergo administrative review by the IRB submission manager in order to determine appropriate IRB and staff reviewer assignment.  In this state, the IRB submission manager can also send administrative issues to the study team. 


    IRB Member Modification Review

    This is a state in ARROW in which an IRB member can conduct a review of a modification response by the study team upon the request of an IRB staff reviewer.  In this state, the IRB member can submit modification review notes regarding their assessment of the modification response.  Then the IRB staff reviewer forwards the submission back to IRB Review of Modification Response.


    IRB Pre-Review

    This is a state in ARROW.  Studies in this state will undergo pre-review by an assigned IRB staff reviewer.  The staff reviewer can perform multiple activities in this state, including sending the application for consultant review, sending the application to ancillary committee, completing pre-review checklist, and scheduling the application for an IRB meeting.


    IRB Review of Modification Response

    This is a state in ARROW.  Studies in this state were previously reviewed by the IRB and modifications were requested.  The response from the study team requires review by the IRB to determine whether the study team has adequately addressed the IRB's requests.  In this state, the IRB staff reviewer can send the modification response to an IRB member or schedule it for review at a full meeting.


    IRB Withdraw

    This is an activity in ARROW that allows the IRB staff reviewer to withdraw a study from the review process.


    IRB Withdrawn

    This is a state in ARROW.  Studies in this state have been withdrawn by the IRB due to lack of response to an IRB request for modifications or deferral response.


    IRBS (IRB Staff Reviewer)

    Abbreviation for IRB staff reviewer role.


    KCS (Knowledge-Centered Service)

    KCS (Knowledge-Centered Service) is a knowledge/information management model. It focuses on knowledge being created, used, reviewed, and edited by the same team that is resolving the issues, rather than a parallel knowledge management team.

    This is an activity which allows a coordinator of any scientific review committee to comment on a submission during their review.  This comment is visible to the study team and the IRB staff.

    Located at 365transition.wisc.edu, the Office 365 Transition Website is the first stop for those who are interested to know more about the transition. It provides general info on everything from the project history and the business case to the Office 365 Team roster and announcements to campus.


    Modification Response Reviewed

    PRIMIPAROUS (cow)

    1. A young cow that is pregnant for the first time. 2. A cow that has given birth once.


    Print Reviewer Checklist

    When a study is scheduled for an IRB meeting, IRB staff reviewers will upload a reviewer checklist summarizing any issues/concerns/questions that the board should be aware of.  The checklist can then be printed out by key board reviewers during their review of the study.  This is where board members comment on affirm/deny staff recommendations.

    Some studies undergo a facilitated review rather than a full review, for example, studies sent to the NCI CIRB.  IRB staff will record the facilitated review decision using this activity.  This activity moves the study to the "awaiting correspondence" state.


    Reviewer Notes

    Reviewer Notes are notes made in an application by IRB staff. Reviewer notes labeled IRB Question require a response from the study team. Only IRB staff can create Reviewer Notes in ARROW.


    Scientific Review

    This is a state in ARROW.  Studies in this state must undergo review by the Institute for Clinical and Translational Research Scientific Review Committee (ICTR SRC), the UW Cancer Center Protocol Review and Monitoring Committee (PRMC) or the Clinical and Translational Research Core  Protocol Implementation Review (CTRC PIR) before moving on to the IRB Pre-Review state.


    SCIREV (Scientific Review)

    An abbreviation for the role of the Scientific Review committee.  Scientific Review Committee activities can only be executed by their respective coordinators.

    Searchable Question (WiSH)

    When marked, enables questions to appear on the Public Website to be used as filters. However, we strong discourage users from making questions searchable. Some question types when made searchable have the capacity to overload the Public Website. Our best practice is to never use the searchable function.


    Send to PRS Review

    This is an activity in ARROW that allows the IRB submission manager to send a submission to Preliminary Review Session (PRS) review. Once the IRB submission manager executes this activity, the submission moves to the PRS review state.

    The phrase "sifting and winnowing" refers to UW-Madison's historic tradition of defense of intellectual and academic freedom.     Stunted growth

    Stunted growth also known as stunting and nutritional stunting, is a reduced growth rate in human development, characterized by a low height for one's age. It is a primary manifestation of malnutrition (or more precisely undernutrition) and recurrent infections, such as diarrhea and helminthiasis, in early childhood and even before birth, due to malnutrition during fetal development brought on by a malnourished mother.     Service Description


    Very Low Density Lipoprotein (VLDL)

    Particles that carry cholesterol and fat throughout the bloodstream. These particles are released from the liver into the bloodstream. They are similar to chylomicrons which originate from the gut because they both carry cholesterol and triglycerides which are gradually released in the bloodstream to be absorbed by body cells along the way. In the process of losing triglycerides, the VLDLs grow smaller and turn into LDLs (Low Density Lipoprotein) which have lost all their triglycerides.

    This is an activity in ARROW that allows IRB staff, IRB members, and study teams to view differences between versions of the submitted smartform application.  For example, if a study team made changes to the initial review application as a result of IRB Pre-Review and submitted those change to the IRB, a new version of the application is tracked.  The study team and the IRB staff can easily view the differences between the old and new version of the application by using this activity button.

    The number of days between calving and first attempt to breed a cow. A typical voluntary waiting period is 45 days. However, it may varies from less than 40 to 80 days depending on farmer’s preference and reproductive protocol in place on the farm.


    W-9 Form

    1. Form W-9 is a commonly used IRS form titled Request for Taxpayer Identification Number and Certification. True to its name, individuals and entities use the form to provide their taxpayer identification number to entities that will pay them income during the tax year. Companies use your W-9 for accounting purposes but doesn't send the form to the IRS. The information is used to prepare 1099-MISC forms at the end of the year.

    Wasting

    Workflow

    A workflow is a series of activities/steps that are necessary to complete a task. Each step in a workflow has a specific step before it and a specific step after it, with the exception of the first and last steps. In a linear workflow, the first step is usually initiated by an outside event. If the workflow has a loop structure, however, the first step is initiated by the completion of the last step. Example workflow: A shared conference room needs to be managed on a first-come-first-served basis. Assigned users would be able to reserve the conference room if it is available. Otherwise, the invitation will be declined automatically by the resource calendar.
