Risk Management - Certificates of Insurance Program - Get a Certificate of Insurance (COI)
A certificate of insurance is a document verifying that UW-Madison, Extension has insurance coverage, who it covers, the effective date of the policy, the types of coverage, dollar amount of limits, and deductibles.
COI's are provided by UW-Madison Office of Risk Management through a request form. You must have a signed contract to submit with the request. No 3rd party affiliated with Extension can receive a UW-Madison COI. For example, 4-H clubs must request a COI from their own insurance provider. Only employee led programs with agreements/contracts signed by a authorized UW-Madison signatory will be accepted by Risk Management.
